An exciting opportunity has arisen to join us with working within our friendly and dedicated Supported Living Service located in Portsmouth.

Outreach Support Worker – Portsmouth (Full time / Part Time) from a minimum of 20 – 37.5 hours per week

 Flexible shifts over a 24/7 week, 365 days a year (incl. weekends, bank holidays and sleep-in shifts on a rota basis)

 Salary £10.50 per hour rising to £10.90 per hour (subject to skills assessment after 6 months)

As an Outreach Support Worker, you’ll be engaging directly with those we support, inspiring fun and creativity whilst encouraging independence and allowing individuals to live happy, healthy, and fulfilling lives. Within this role, you will be providing morning and evening support to individuals, encouraging their independence by helping them complete day-to-day tasks such as cooking, personal care, light housework, and life management.

The role:

  • Supporting individuals to meet their goals and outcomes in health, well-being, and independence
  • Assisting individuals with their personal, social, and healthcare needs whilst maintaining accurate records
  • Supporting individuals to attend university and complete education
  • Assisting individuals in completing day-to-day household tasks including cleaning, and food shopping
  • Building positive working relationships with family members, carers and other professionals
  • Supporting in organising recreational and social activities giving assistance where necessary in the organisation of transport
  • Assisting individuals to organise and participate in mealtimes, including the preparation of meals, setting, and clearing tables whilst supporting to ensure good food safety hygiene practices are followed

What are we looking for?

  • Motivated, honest, and approachable individuals who are great at building and nurturing relationships
  • Experience of providing high levels of care and support to those with learning disabilities in a social care setting
  • Aspirations to learn and develop skills within the social care industry!
  • The flexibility to work shifts 24/7, 365 days a year including weekends and bank holidays as well as sleep-in shifts on a rota basis
  • A Level 2 NVQ in Health and Social Care is desirable but, training will be provided
  • A full UK driving licence is preferred but not essential (in-work travel paid)
